Dr. Rebecca Lang is a FMCSA (Federal Motor Carrier Safety Administration)–certified medical examiner and is fully licensed to perform the physical examinations required to obtain or maintain a Commercial Driver’s License (CDL). Completed DOT Physical Exam form. This can be found at here (link: https://www.fmcsa.dot.gov/regulations/medical/medical-examination-report-form-commercial-driver-medical-certification). If you cannot complete this prior to your visit, please call the office to arrange doing this in office prior to your visit.
Required for all DOT physicals:
- Valid government-issued photo ID
- Glasses, contact lenses, or hearing aids if used
- A list of current medications (including dosages and prescribing provider)
Additional documentation may be required if applicable:
- Sleep Apnea: if you have sleep apnea, please bring a printout of the past 3 months of CPAP usage data demonstrating compliance.
- Blood Thinners: If you are taking blood thinners such as Coumadin or Warfarin, please bring documentation from the managing provider confirming that your condition is currently well managed.

- Diabetes: If you have diabetes, please bring recent hemoglobin A1C results. If you are treated with insulin, please have the managing provider complete the following form: https://www.fmcsa.dot.gov/regulations/medical/insulin-treated-diabetes-mellitus-assessment-form-mcsa-5870
- Vision Requirements: If you have monocular vision or do not meet standard DOT vision requirements, please bring the completed Vision Evaluation Report Form: https://www.fmcsa.dot.gov/regulations/medical/vision-evaluation-report-form-mcsa-5871
