Who needs a Commercial Drivers License?
Commercial drivers licenses are required for any person transporting hazardous materials, driving a vehicle that is over 26,000 pounds and/or capable of carrying 16 or more passengers.
How long is my appointment?
Appointments for this physical are booked for 45 minutes and will include a review of health history, vision and hearing screen, urine testing, full vital signs, and a physical exam.
How often do I need a DOT Physical?
Depending on health status, this physical needs to be completed anywhere from every 3 months to every 2 years.
What do I need to bring with me to my appointment?
We ask that patients please fill out section 1 of the physical exam form PRIOR to their appointment. This can be found at https://www.fmcsa.dot.gov/regulations/medical/medical-examination-report-form-commercial-driver-medical-certification.
What does a DOT Physical Cost?
The physical exam is $200 due at the time of the visit and is not billable to commercial insurance
