Our electronic medical records now give you the option to easily access your medical records, treatment plans and vaccine record in addition to being able to communicate with your provider or our staff. To access your Personal Health Record, enter the Patient Health Portal by clicking the link below.  You will then be asked your account name and password.  If you do not have a user name and password, please call the office to have one set up for you. Please note, this is only available for established patients.
